In its inquiry into the tragedy on board INS Sindhuratna, the Indian Navy has said that a human error led to the incident. CNN-IBN reported that the probe conducted by the Navy had said that human error led to the fire on board the submarine. PTI[/caption] The probe also said that there had been a deviation from standard operating procedure, though the details of what it was wasn’t clear immediately. Two officers were killed as a result of the fire on board the submarine. Admiral DK Joshi had also tendered his resignation following the incident.
